For individuals seeking medical treatment in the Federal Republic of Germany or applying for a Medical Treatment Visa, several documents are required. These documents ensure a smooth and efficient visa application process and facilitate the arrangement of necessary medical care in Germany.

Required Documents for a Medical Treatment Visa for Germany:

  1. Fully Completed Application Form & Declaration: A meticulously filled-out application form, along with a signed declaration, is required for the visa application process. This form provides essential information about the applicant and the purpose of their visit.
  2. Passport Photographs: Two passport-sized photographs that adhere to strict specifications must be submitted. These photographs play a crucial role in the identification of the applicant and are an essential part of the visa application package. (strict requirements, see this page for details)
  3. Valid National Passport: A valid national passport with a minimum validity period is necessary for the visa application. The passport must have been issued within the last 10 years and should have at least 12 months of validity remaining after the scheduled return date.
  4. Confirmation Letter from Health Insurance: A confirmation letter from the applicant’s health insurance provider is required. This letter must state coverage for emergency medical or hospitalization expenses, with a minimum coverage amount of 30,000 Euros. It serves as proof of the applicant’s ability to cover any medical expenses during their stay in Germany.
  5. Travel Arrangements: Proof of travel arrangements, such as flight reservations, rail tickets, or other transportation details, must be provided. These documents demonstrate the applicant’s intent to travel to Germany for medical treatment and their planned itinerary. (Flight reservation, rail tickets, others.)
  6. Personal Covering Letter: A personal covering letter outlining the purpose of the applicant’s stay in Germany and detailing their entire travel plan, including accommodation arrangements and medical treatment appointments, is required. This letter provides additional context to the visa application and helps authorities understand the applicant’s intentions.
  7. Leave Letter (If Employed): If the applicant is currently employed, a leave letter from their employer is necessary. This letter confirms the applicant’s employment status and states their intention to take leave for medical treatment purposes.
  8. Certificates from Medical Doctors: Certificates from medical doctors play a crucial role in the visa application process. These certificates include:
    • Certificate confirming the need for specific medical treatment.
    • Certificate confirming that necessary treatment can be provided, along with an appointment confirmation.
  9. Proof of Advance Payment/Funding: Proof of advance payment for medical treatment or evidence of financial arrangements with medical practitioners in Germany is required. This proof ensures that the applicant has made adequate financial preparations for their medical care.
  10. Correspondence Between Medical Practitioners: Correspondence between the sending medical practitioner (from the applicant’s home country) and the receiving medical practitioner (in Germany) may be requested. This correspondence provides additional information about the medical treatment plan and facilitates coordination between healthcare providers.
  11. Proof of Accommodation: Evidence of accommodation arrangements for the entire duration of the applicant’s stay in the Schengen area must be provided. This proof ensures that the applicant has suitable lodging during their medical treatment period.
  12. Proof of Civil Status: Documents proving the applicant’s civil status, such as a marriage certificate, birth certificate of children, death certificate of spouse, or ration card (if applicable), are required. These documents serve as additional identification and provide relevant personal information about the applicant.
  13. Proof of Financial Status: Depending on the applicant’s employment status, proof of financial status must be provided. This includes:
    • For employed individuals: Payslips of the past three months or an employment contract.
    • For self-employed individuals: Certificate of Proprietorship or other proof of ownership (e.g., proof of land title, proof of income from agriculture).

Where and How to Apply for a Medical Treatment Visa:

Applicants should apply at their home country’s German Embassy or Consulate. First-time applicants are required to submit the application form and all required documents in person. However, frequent travelers may choose to submit their application and documents via an authorized agent or a reputable travel agency.
It is recommended that applicants apply for a visa at least 3 weeks before their planned travel date to account for any processing delays and ensure timely approval.

Payment for the Medical Treatment Visa

Applicants are required to pay the visa fees through a bank transfer. Checks or credit cards are not accepted for visa fee payments. The money order must be made in the name of the German Consulate in the applicant’s home country and must not be older than two months. As of 2008, the cost of a visa is 60 Euros.

Important Note

It’s essential to note that all documents must be presented in the given order in their original form. If applicants wish to retain any originals, they must provide copies (A4 size). Additionally, the German Missions reserve the right to request additional documents or schedule a personal interview in some cases. Submission of the above-mentioned documents does not guarantee visa approval. Incomplete documentation or refusal to appear for a visa interview may lead to the rejection of the application.
